Identity Theft Insurance: Summary of Benefits*

Identity theft insurance provided under policies issued to our third party vendor provides reimbursement, up to $1M with $0 deductible, for expenses associated with your identity theft recovery. Financial reimbursement covers your costs incurred from your identity recovery including these and more:

• Lost wages or income
• Attorney and legal fees
• Expenses incurred for refiling of loans, grants and other lines of credit
• Costs of childcare and/or elderly care incurred as a result of identity restoration

Coverage is subject to the conditions and exclusions in the Summary of Identity Fraud Reimbursement Coverage. Please read the Summary of Identity Fraud Reimbursement Coverage carefully to determine rights, duties and what is and is not covered. Coverage is provided to residents of the United States and U.S territories. To report an incident, contact Complete ID support at 855-591-0202. In the event there is a related insurance claim, the claim will be handled by the insurance carrier.
